Request a copy of "Prescribing Controlled Substances in Utah" by email

The Utah Medical Association will email a copy of this booklet free of charge to physicians (MDs and DOs) and physician assistants (PAs) who request one.  Any others who desire a copy must purchase one for $5 by sending a check to the UMA office along with a note as to where the publication is to be sent.
